Have you configured the accounting
aaa group server tacacs+ TACACS_GRP
server name TACACS_SERVER
aaa accounting exec default start-stop group TACACS_GRP
aaa accounting commands default start-stop group TACACS_GRP
I have checked the configs once again on my Cisco devices, every one of them is sending the Remote IP Address. I have tacacs accounting enabled in all my switches as shared above.

Cisco switches and out of 10 request authentication is failing due to this reason for couple of request .

Tacacs has accounting as well. So check if it's enabled there as well.
What type of switch are you using?

Check if you have Radius Accounting enabled on the switch from where this Request is coming from.
